Information for Illinois Residents

Illinois Administrative Code Section 2028 provides protections for victims of actual or threatened violence, including domestic violence. A victim, the legal representative of the victim, or a child victim’s parent or guardian may request to receive policy information at an alternate address, telephone number, or other method of contact.
 
Upon delivery of a valid order of protection or a reasonable request for confidentiality, the insurer is prohibited (for the duration of the order or request) from disclosing to the policyholder or other person the address, telephone number or any other personally identifying information of the insured, of any person or entity providing health services to the insured, the nature of the health services provided, or any other information from which there is a reasonable basis to believe the forgoing information could be obtained. If a child is a covered person, then the right established by this section may be asserted by the child’s parent or guardian.
 
The Hartford has adopted procedures to maintain the confidentiality of, and to refrain from disclosing, the information protected by the above statutes. Information for New Mexico Residents

New Mexico law provides victims of domestic abuse with important protections. The Hartford has adopted procedures to maintain the confidentiality of, and to refrain from disclosing, the information protected by this law.
 
You may report to The Hartford that you are a victim of domestic abuse and wish to be a protected person under New Mexico law. In addition to receiving information from you about your status as a victim of domestic abuse, we may also receive information about you from other people or entities, such as medical professionals, law enforcement agencies, or others. We may collect information about whether you have experienced domestic abuse; your home or work address or phone number; and the status of other people who may have a direct, close, or personal relationship with you, such as your family members or employer. This information may be collected during the processing of your claim.
 
You have the right to access, correct, amend, or delete any information we receive regarding your status as a victim of domestic abuse. You also have the right to request that we keep your address and phone number confidential and protected. Under New Mexico law, insurers are prohibited from using information about your status as a victim of domestic abuse to:
 
  • Deny or refuse to issue, renew, or reissue your policy,
  • Cancel or terminate your policy,
  • Restrict or exclude coverage or benefits of your policy, or
  • Charge a higher premium for your policy.
We may share information relating to you as a victim of domestic violence in the following circumstances:
 
  • With you or an individual that you have specifically designated in writing,
  • With a healthcare provider for the direct provision of healthcare services,
  • With a licensed physician that you have designated,
  • With your attorney,
  • Pursuant to a court order or as otherwise required by law, or
  • For specific business purposes, such as with our vendors to administer our policy.
We may also share your information with insurance support organizations, who may retain your information to prepare reports for us. Insurance support organizations may not disclose your information to other individuals or entities without your written consent except as listed above.
